Welcome to Collegiate Academy!

Please review the Parent Enrollment Checklist linked below. When applying for enrollment, please complete all required forms and submit all required documents listed below. Jeffco Second Round Choice Enrollment Form (Paper Form)
Student Transcript/Report Cards
Attendance Report
Behavior Report

Required for Applicants Outside Jefferson County:

Jeffco Application for Non-Resident Admission (Paper Form)
Jeffco Application for Non-Resident Admission (Online Form)
Jeffco 18-19 Choice Enrollment Application (Paper Form)

Required for students with an IEP, ALP, or 504 Plan:

Current IEP & Original Staffing Report
ALP Plan
504 Plan

2018-2019 Mid-Year Transfer

An official Admin Transfer Form will be required for all mid-year transfers after September 1. Request from your current school.  

2018-19 Enrollment

First round choice enrollment closed on January 31, 2018. We are currently in the Second Round Choice window and are scheduling tours and accepting new students on a space available, first come, first served basis.

Open enrollment closes September 30, 2018; requests for enrollment after this date will be reviewed on a case by case basis. For more information, contact Rosalyn Vette, Enrollment Coordinator, at rvette@collegiateacademy.net.
